By clicking the link above, you will open our demonstration site in a new browser window. Your customized site will contain your logo, facility name, etc. If your facility has a website, your webmaster can create a link to your personalized MD-Query site.

Pretend you are a user and enter your name, facility and address. This information will be used to print the verification letter and to track the access.

The login ID is demo (all lowercase) The password is mdstaff (all lowercase)

Click on the check box to accept the terms and conditions. By clicking the "Submit" button, you will be directed to a search page that is used to locate a specific physician. Please use the following person as an example:

Last Name: ackerson First Name: paul SSN: 5786

Once the physican is located, click on their name to load the verification letter. At this point you can view and print the letter.
